A New York healthcare network is currently seeking a new, licensed Registered Nurse (RN) to join their team in Dutchess County as a Case Manager.

Responsibilities:
The Case Manager (RN) will:
Provide ongoing assessment, planning, implementation and delivery of evidence based interventions Develop and facilitate collaborative partnerships with all members of the care team Participate in and presents cases at interdisciplinary care team meetings to discuss changes in health status and/or level of care and coordinate interventions determined appropriate by the team Develop and update holistic care plans that addresses members' needs Ensure that the member has a seamless transition from one level of care to another based on regulatory requirements and department policy and procedures Identify and track all Advance Care Directives discussions and placement Ensure proper utilization management of skilled services and level of care changes
Qualifications:

years of Nursing experience Graduate of an accredited School of Nursing NYS Registered Nurse (RN) license Desired Skills:
Bachelor's Degree in Nursing Case Management and/or Utilization Review background Experience in a Long Term environment Recommended Skills Case Management Certified Nurse Practitioner Nursing Regulatory Requirements Utilization Management Partnerships Estimated Salary: $20 to $28 per hour based on qualifications.
